Ankle fractures classified as Weber C (fracture line in the fibula above syndesmosis) are associated with the lesion of the syndesmosis ligaments. These are uncommon lesions, caused by high-energy trauma to young patients. Its treatment still today generates controversy among orthopedists, being imperative the surgical approach with correction of the length and rotation of the fibula through the reduction and fixation under direct visualization of the distal tibiofibular incision. We will present a case report of a young patient with Weber C ankle fracture with syndesmosis lesion, and the surgical treatment through open reduction and internal fixation with plates and screws.
