Abstract

Sacralectomy is a complex surgery of a complex anatomical region that requires a multidisciplinary team trained and experienced team. The major indication is the resection of malignant tumors of the sacrum, the most common is chordoma. In rectal or pelvic tumors, it is also a procedure indicated due to proximity or sacral involvement. The great differential of this procedure and the level of the osteotomy of the sacrum is that lesions above the sacroiliac (S2) line, may cause permanent neurological sequelae of the sphincters and even impotence in the male patients. Already for the osteotomies below S2 (preserving roots of S2) the lesions are temporary or even nonexistent. Dural sac ends between S2-S3. When there is anterior lesion an approach should be performed anteriorly and posteriorly. If the mass is located posteriorly this approach is chosen.
