A 17-year-old female with bilateral congenital clubfoot was submitted to two postmedial release approaches on the right foot, and a posteromedial release approach in the left foot, followed by Ponseti treatment after recurrence of the foot left. Correction of the deformity was obtained in adult cavus varus and equino with six changes of casts for the left foot. Even with plantigrade feet, the patient developed foot pain, especially on the right, which gradually increased and became incapacitating, even at rest. The right pain was more important and located in the anterior region of the ankle. Then, an arthroscopic approach was performed to treat anterior tibial talar impact with pain improvement for a few months. It progressed in three years with worsening of the pain, now referred to the subtalar region and more generalized. The patient undergone surgery for triple foot arthrodesis.
