Position tibial tendon transfer to the third cuneiform through the interosseous membrane is described. A patient with cavus neuromuscular foot with progressive deformity was operated, he had important gait alteration and difficult to wear regular shoes. The foot was aligned by bony corrections and balance with posterior tibial tendon transfer.
