Thumb amputations can compromise up to 50% of the hand's function. Currently, toe transplantation is the only safe and reliable procedure that replaces "equal for equal" cases of absence or loss of the thumb and fingers. We describe a case of transplantation of the 2nd toe to a disarticulated thumb at the level of the metacarpophalangeal joint with a good functional and aesthetic result and without important losses to the donor foot.
